2021-2022 School Year
Students who will be starting grades 9 and 11 are required by NYS law to have a physcial within the first 30 days of the start of school. Please click on your child's grade for more information and forms. This rule pretains to both in person and remote learning. Click on your child's grade to acess physcial forms.
12th grade students who have not yet met their meningitis vaccine requirement, must show proof of vaccine prior to start of school.
NURSE'S OFFICE SUMMER HOURS -2021
The Nurse's office will be open on a part-time basis in August.

It is encouraged that parents use these hours to bring up student's medical forms and any medication needed for the new school year. This will alleviate your child coming to nurse's office the first week of school, which we anticpate to be extremely busy.
Also, feel free to use these hours to come up to our office and discuss any medical issue about your child.

Asymptomatic travelers entering New York from another country, US. state, or territory are no longer required to test or quarantine as of April 10,2021.Quarantine consistent with the CDC recommendations, is still recommended for all travelers who are not fully vaccinated or have not recovered from laboratory confirmed COVID 19 during the previous 3 months.

NURSES OFFICE VISITS
In the past students would come to the nurse's office for various reasons and able to return to class. The "new normal" for the nurse's offfice is that a student who comes to our office with any COVID -like illness will need to be picked up by parent or guardian right away and will be given a form with instructions to return to school.
We understand these are very difficult and stressful times for all but we need to follow these protocols to keep everyone safe. Our goal is to ensure the safety of everyone in the building and we greatly appreciate your help in this matter.
Please click on the appropriate tab of the grade your child will be entering in the new school year. Fill out the required forms above and return to the nurse's office prior to the start of school. It is imperative we get these forms back ASAP.

Scoliosis- Grade 9 boys
Hearing - Grades 11 and new entrants
Vision - Grades 11 and new entrants
The results of these screenings will be recorded on each student's health record. Parents will be advised in the event a referral to a specialist is needed.
In order for a student to have a gym exemption, a note from his/her doctor is required which should include dates of the exemption. A follow-up note will then be required to return the gym class. ALL GYM NOTES MUST BE HANDED IN AT THE NURSES OFFICE.
Any student on a long-term gym medical will be required to update their exemption during the time of the medical.
1 Varicella (chicken pox) high school born on or after 1994, 2 varicella vaccine entering 9th, 10th, 11th, and 12th grade.

2 Measles AFTER the age of one year old
1 Mumps & Rubella-these may be given as combined MMR

Meningococcal conjugate vaccine for grade 12: 2 doses or 1 dose if the dose was received at age 16 years of age or older
