    It is important to remember that good physical 
    and mental health are essential for every child's
    educational development and 
    achievement

     

     2023-2024 School Year

     

    Students who will be starting grades 9 and 11 are required by NYS law to  have a physical within the first 30 days of the start of school.  Please click on your child's grade for more information and access forms. 

     

    12th grade students who have not yet met their meningitis vaccine requirement must show proof of the vaccine prior to the start of school.

     

    Parent/Guardian, please click on the appropriate tab of the grade your child will be entering in the new school year. Fill out the required forms above and return to the nurse's office prior to the start of school. It is imperative we get these forms back ASAP.

    ANNUAL SCREENINGS
    New York State Education Law, effective 7/1/2018, requires the following annual screenings for high school students.
    Scoliosis - Grade 9 boys
    Hearing - Grades 11 and new entrants
    Vision - Grades 11 and new entrants 

    The results of these screenings will be recorded on each student's health record. Parents will be advised in the event a referral to a specialist is needed.


    GYM EXEMPTIONS
    In order for a student to have a gym exemption, a note from his/her doctor is required which should include the date of the exemption.  A follow-up note will then be required to return to the gym class
    **ALL GYM NOTES MUST BE HANDED IN AT THE NURSES OFFICE**
    Any student on a long-term gym medical will be required to update their exemption periodically.
