• INTERSCHOLASTIC SPORT AND MEDICAL INFORMATION

     

    In order to be eligible to participate in our Interscholastic Athletics program at Great Neck South High School a student must have an up to date physical ("A" Form) and a sport/season specific Family ID registration prior to the start date of their chosen sport.
    For tryouts students are required to bring the coach a copy of their schedule with student ID number.

    "A" Form –The physical examination may be performed by a medical doctor, a physician’s
    assistant or a nurse practitioner. The HEALTH HISTORY on form A must be COMPLETED and SIGNED by a parent/guardian.

    All "A Forms" for sports must be submitted via Family ID.
