The B form is complete only if ALL questions are answered YES or NO
Any question answered YES must be explained.
Give details about what happened? When did it happen? Is it resolved or does the athlete have any restriction?
The B form can only have one sport listed.The B form must be signed by a parent or guardian.

When To Keep Your Child Home From School
Cold and Flu Season are here. Your child should stay home if they have any of the following symptoms:
Fever of 100F or greater without the use of fever-reducing medication
A cough
A sore throat
diarrhea/vomiting
Any other flu-like symptoms
Any student who comes to school with any of these symptoms will be sent home.
If your child is diagnosed with any communicable disease, please report this to the nurse's office immediately

Please click on the appropriate tab of the grade your child will be entering in the new school year. Fill out the required forms above and return to the nurse's office prior to the start of school. It is imperative we get these forms back ASAP.

Scoliosis- Grade 9 boys
Hearing - Grades 11 and new entrants
Vision - Grades 11 and new entrants
The results of these screenings will be recorded on each student's health record. Parents will be advised in the event a referral to a specialist is needed.
GYM EXEMPTIONS
In order for a student to have a gym exemption, a note from his/her doctor is required which should include dates of the exemption. A follow-up note will then be required to return the gym class. ALL GYM NOTES MUST BE HANDED IN AT THE NURSES OFFICE.
Any student on a long-term gym medical will be required to update their exemption during the time of the medical.
REQUIRED IMMUNIZATION
1 Varicella (chicken pox) high school born on or after 1994, 2 varicella vaccine entering 9th,10th and 11th grade.
3 of each..DPT, Polio, Hepatitis B
2 Measles AFTER the age of one year old
1 Mumps & Rubella-these may be given as combined MMR
1Tdap ALL STUDENTS ENTERING 9TH-11TH GRADES FOR 2012-2013 SCHOOL YEAR
Meningococcal conjugate vaccine for grade 12: 2 doses or 1 dose if the dose was received at age 16 years of age or olderUNDER THE "MORE" TAB YOU WILL FIND INFORMATION REGARDING KEEPING MEDICATIONS IN THE NURSE'S OFFICE FOR YOUR CHILD, AS WELL AS INFORMATION ON WORKING PAPERS AND CONCUSSION PROCEDURE.
