SUMMER 2019 IMPORTANT MESSAGE
Please check the important forms page for the required forms due to the nurse prior to the start of the school year. They are organized by grade so please check carefully as information required by grade varies.
Fall Interscholastic Teams (Grades 7 & 8)
In order for your 7th or 8th grader to be considered for any of our Fall Interscholastic Teams, your child must return to our school nurse, Lisa Farazmand, the “A” and “B” Medical Forms completed by his/her doctor and signed by either a parent or guardian. Click here for instructions on how to complete the forms.
A forms can be submitted any time as long as the date of exam is no earlier than 9/1/18. The B forms can be submitted to the Nurse’s Office beginning on Thursday, August 8th. Forms can be mailed or dropped off at the Nurse’s Office. Our Interscholastic Program starts on Thursday, September 5, 2019. Practices are scheduled daily from 3:00 p.m. to 5:00 p.m.
Deadline for submission of forms for the Fall Sports is Monday, August 19th.
Please note the school doctors will be available on August 26, 2019 from 11am-1pm in NORTH MIDDLE SCHOOL to perform Sports Physicals, free of charge, for student trying out for sports in September. This is only for those students who do not have a valid physical and are not able to get a physical from their own pediatrician. About Ms. Farazmand
Mrs. Farazmand has been a School Nurse in GNPS since 2011 and here at North Middle School since 2014. She earned her nursing degree from Nassau Community College and is currently working toward her Master’s degree at Molloy College. She has experience in many different fields of nursing including Gastroenterology, Medical/Surgical nursing, Oncology, Camp Nursing, ENT and Allergy/Immunology. School nursing is not just her job, it is her passion.
Ms. Farazmand has lived in Great Neck along with her husband and three children since 2006. Student safety is her top priority. Students’ overall health and well being have proven to be critical in the learning process. Mrs. Farazmand, along with the North Middle School team including administrators, guidance staff, deans, teachers, and support staff works hard to promote a physically, emotionally, and socially healthy environment for our students to ensure their academic success.
