    Alumni Transcript Requests:

    Step 1: Complete the attached form here.

    Step 2: Ensure that you sign the form. Requests cannot be processed without a signature from the student/alumni requesting the transcript.

    Step 3: Submit the completed and signed form to Sarah Scala in the Guidance office either in person, mail, fax (516-441-4895), or by email (sscala@greatneck.k12.ny.us).
